We require that each wholesale broker be appointed. It's easy:

1. Download & complete the Premium Agreement
2. Download and complete the W-9 form: http://www.irs.gov/pub/irs-pdf/fw9.pdf
3. Submit with a copy of your license and E&O Declarations Page to your local West-Pro office
4. Please don't forget to review our Privacy Policy
5. Please review our position on Broker of Record requests
6. Pay accounts via Fax or E-Mail

Once we receive the completed paperwork, We'll contact you to assist you in developing a positive and rewarding relationship with West-Pro.
